From July 2024, Sam Whittle, a Diabetes Specialist Nurse (DSN) from the Royal Bournemouth Hospital is integrating into the HealthBus clinical team one day a week, which is a transformative step for the care of homeless patients. Homeless individuals face significant challenges in managing diabetes due to unstable living conditions and limited access to healthcare. A DSN brings specialised expertise, ensuring accurate diagnoses, effective treatment plans, and continuous monitoring to prevent complications.
Additionally, Sam can provide tailored education, teaching patients to manage their condition in their unique circumstances. This includes managing blood sugar levels, understanding the importance of regular foot care, and the role of diet and exercise. Empowering patients with this knowledge is crucial for long-term disease management.
The holistic approach of the HealthBus, combined with the DSN’s comprehensive care, addresses not only medical needs but also psychosocial factors like mental health and substance abuse. The DSN can act as a liaison, ensuring coordinated care between the HealthBus and other healthcare providers, reducing fragmented care.
